Insurance/Billing Information


If we participate with your insurance, all co-pays are due at the time of treatment. This is a requirement of your health care plan. Co-pays not paid at the time of service are subject to a $15 service fee.

By participating with your insurance, we may accept assignment of benefits after determining their reimbursement. However, you must provide us with accurate insurance information, and you are responsible for all balances whether or not your insurance company pays. If we do not accept assignment of benefits, then you must pay in full at the time of service.

Our practice is committed to providing the best treatment for our patients and we charge what is usual for our area, but it is possible that your insurance company will decide that some or all of the services provided are not reasonable and necessary. Our billing staff will work with you and your insurance company to answer questions and file claims, but ultimately the parents or adult accompanying the child are responsible for full payment. We accept cash, checks, Visa, Mastercard, Discover and American Express.

There will be a $50 fee for all returned checks

We require you to review and sign a statement of our Financial Understanding prior to any treatment. All parents/guardians of our patients must complete our Information and Insurance Form before seeing the doctor.

Late Cancellations and No Shows

Each time a patient misses an appointment without providing proper notification (no shows); another
patient is prevented from receiving care. If you have 3 or more no shows within a calendar year, it may
result in your child’s discharge from our practice. Due to high patient demand for appointments, and
limited availability, any missed appointments or cancellations without 24 hour advance notice is subject to a $50 fee. These fees pertain to all insurances, including Medicaid and Child Health Plus.
